      <image:title>Physical Therapy Blog - The Biggest Mistakes Runners Make Returning from Stress Fractures</image:title>
      <image:caption>Bone stress injuries (BSI’s) refers to overuse injuries to bone (such as stress fractures and stress reactions). Running is one of the most common sports that we see BSI’s in because of the repetitive nature of the sport. Research has shown that 4-5% adolescent cross country male and female athletes experience a BSI annually. In elite athletes, 20% sustain a BSI annually. While the terminology can be confusing, there is a differentiation between stress reactions and stress fractures. A stress reaction refers to a bone injury that presents with localized edema on MRI, but without evidence of hairline fracture. A stress reaction can progress to a stress fracture where we see a visible hairline fracture on MRI. Treatment usually involves 8-12 weeks of non-weightbearing (depending on location and degree of injury). Think about that for a minute… 8-12 weeks of non-weightbearing and zero impact for a runner is a long time. A BSI is your body telling you that the stress of training you’ve put on it for several weeks have caused the bone to inadequately remodel and repair itself. There is a shift in activity between the osteoclasts that breakdown bone and the osteoblasts that rebuild it. This is a naturally reoccurring process in the body. If the recovery in training doesn’t match the intensity, the osteoblasts can’t rebuild quickly enough. If you have had ONE bone stress injury, your risk of having another increases 6x.</image:caption>

      <image:caption>Our pacer got me across the half in 1:24.40. Funny enough, this is the 4th time I've ran 1:24 in the half. The first 2 times this year, I was actually racing a half marathon. 1:24 is also my current PR. The 3rd time was during marathon season and the 4th time was during this marathon. I couldn't help but think "damn, 1:24 actually feels good". I had some thoughts that maybe I had gone out a little spicy. By mile 14, it was me and the 2 davids (my training buddy David Lam and our pacer David Clarke). A tale of 2 davids perhaps? I felt like I was working by mile 16-17 and knew this was going to be a hard day and that I was going to have to work for it, possibly pay for it. My body felt the beating from the first half of the course. Mile 20, my pacer had to drop out. It was his 3rd marathon in 3 months so he was either going to have to drop at 20 or run the whole race. Believe me, I practically begged him to stay! Even though I was falling off pace, he was reeling me back in. "Trust your training. Stay IN the race." is what I had to tell myself. I knew it was going to be one of the most painful races I've ever run, but I wasn't giving up yet. My legs were fatiguing and ankles didn't feel springy.</image:caption>

      <image:caption>I ran the first 2 days I was symptomatic, but only because I tested negative and didn’t know it was covid. I continued to walk for exercise to get some fresh air. I never felt bed ridden. I tried to run on day 5, 30 minutes (very easy 9:15 pace) and my heart rate was almost 150. Normally, my heart rate is in the low 130’s at that pace so I knew it was out of the norm. I called my coach Mario and we both decided I would scratch out of Berlin Marathon and focus my efforts into getting healthy for CIM at the end of the year. It was a hard pill to swallow. Perhaps if it was just covid, I would still do it. Coming off a post tib injury made it complicated. Even then, I was scared at how taxing running a marathon (even an easy one for me) would be on my immune and cardiovascular system. It was logical to skip it.</image:caption>

      <image:caption>Race weekend arrived and I realized that for the FIRST time, I did not have to get on a plane and travel. One mistake I made in Chicago was I spread myself out too thin by cramming in social runs, expos, meetings, phone calls, etc that I wasn’t taking care of myself. This time, I spent the evening with my boyfriend in our hotel having dinner and facetiming with my niece. For once, I had the best sleep before a marathon. I’m historically a terrible sleeper due to race day nerves and fear I’ll sleep through my alarm. Since it’s my 3rd time getting ready for a marathon this year, I had nothing to worry about. But deep down, I had that “what if” feeling. What if my hamstring or my knee doesn’t hold up tomorrow?</image:caption>

      <image:caption>Driving the northern ring road was beautiful. Two way roads which sometimes turned into one lane to cross a bridge, and the opposite side yielding. Sometimes the road was paved and then turn into gravel. Tunnels that spanned for several miles, some even only had 1 lane so cars heading east had to pull over and yield. Once we past the West Fjords, the weather flipped and we had sunny blue skies. We arrived in Siglufjordur at 5pm to pick up my race bib at Segull Brewery where I met my friend &amp; photographer Jordan, his wife Michelle, and their daughter. It was such a small race that I also met our race directors Gestur and Helga. It was funny to finally meet the two people I had been emailing all the way from America to ask how runnable this course even is.</image:caption>

Day before a race?” Normally, I only have water and electrolytes for 10 days before a race. I was here to have fun and experience trail racing so of course I went for the beers. I also want to mention that the Icelandic people are extremely kind and welcoming to visitors. We never had an unpleasant interaction!</image:title>

That’s the thing with small fishing towns in the North, you can get everywhere in 5 minutes. Luckily, my race didn’t start until noon on August 28th so I slept 11 hours to compensate for major jet lag. We had a delicious breakfast at the hotel buffet before Stephenie dropped me off at the race start in Olafsjordur. Trail running gets another brownie point there: a normal, full breakfast. And wow if I can sleep like that every night before a road marathon, I’d be way more relaxed.</image:title>

      <image:title>Physical Therapy Blog - North Ultra 16mi Race Recap - I thought the wind was bad going up the pass since I was going against it, but it was actually worse coming back down against my back. It was so strong that it pushed me around to the point where I didn’t know where my feet would land. The descent was rocky with larger lava type rocks. We ran right next to a stream with tall grass. Beware though - the tall grass looks pretty, but it was treacherous as I couldn’t see anything underneath. Because of all the glacier melt, the valley has many cracks in the ground with mini streams and mud. This part of the course, I felt like I was steeplechasing over muddy streams. Even more difficult was the rolling nature of this part of the course. Running down a hill, steeple jumping a muddy stream in the middle, to land on the other side and running uphill was the new name of the game. It was this x10, with gnats in my face.</image:title>

I didn’t care that my shoes were extremely soaked and my feet were cold. I actually had a lot of pent up energy from having to run/hike so many sections of the pass. I wanted to get a workout and see how fast I could run the last stretch. I started running marathon pace and it felt good!! I also passed a significant amount of people on this stretch. All of whom, I said “good job” too, but I assume they didn’t understand me (language barrier).</image:title>

      <image:title>Projects - Camp Renegade</image:title>
      <image:caption>Camp Renegade, hosted by Renegade Running, was a week long, free running camp for high schoolers in the Bay Area. We hosted 50 runners and welcomed all abilities. The event was sponsored by Nike Running, which provided funding for catered lunch, snacks, buses to transport the runners on a daily basis, and really cool swag for the runners to keep. I’ve lived in many metropolitan cities now and have immersed myself in the running culture. What I’ve seen is that youth who grow up in cities designed for running (i.e. Boulder, Portland, Flagstaff) have easier access to running camps and expert resources. Through those experiences, they are at an advantage to train at a higher level than those in lower income communities where not much funding goes into sports. Renegade Running found a way to make running accessible to high schoolers by making the camp completely free. To get a better glimpse of what Camp Renegade included, check out the website itinerary Each day had its own agenda and buses transported the runners to a different location (berkeley marina, Laney College Track, Redwood Regional Park for trail running, and the Polo Fields at Golden Gate Park). Running was only one component of the camp though. The high schoolers had a 1 hour injury prevention talk with me, had a session with an academic advisor on applying to college, and getting to know local elite runners in the community.</image:caption>

      <image:caption>It was truly a privilege to be the guest speaker on injury prevention. While I thought for many years, it’s important to teach the younger generation of runners how to “prevent” injury, the truth is that we can’t prevent all of them. My approach this time around was Injury Preparation. How can I normalize the talk of running injuries to this group of 50 high school runners, teach them to communicate about their pain, and when to seek help. I started by asking the group if they knew what physical therapy was and what we do. About 30% of the high schoolers knew. Not bad - I couldn’t tell you what a PT did when I was 16. Many have access to athletic trainers, but it was important to me to distinguish the role of each health care professional. I then did an ice breaker where I asked the students to raise their hands if they’ve had “x” injury. About 75% of the students have had shin splints and 75% have all experienced some type of knee pain. I think the students find comfort in knowing and seeing how normal it is to experience these injuries.</image:caption>

      <image:title>Projects - Oakland Tech High School Injury Prevention Workshop - For Fall 2022 cross country season, I was invited to be the guest speaker for Oakland Tech High School. With local community help, I donated 30 resistance bands to the program so they have the equipment they need to do their exercises. We talked about the most common running injuries and almost every high school athlete raised their hand when I asked about shin splints. The most important thing for high school athletes to understand is differentiating between pain from injury vs soreness. There’s also pain you can run through (&lt; 3/10) and pain you should not run through (&gt;4/10, limping, swelling, sharp). Communication between athletes and coaches is critical as I know athletes are often scared to tell their coaches they have pain out of fear that they will be told to rest. Rest isn’t always the solution and your coach can help you modify your running early on so your injury doesn’t progress.</image:title>

We focused on our technique and what compensations to avoid with each exercise.
